Scheduling

Contact the practice to request a visit. Appointment times are a limited resource, so book as early as you can.

Arrival & Check-In

Arrive on time and bring a current insurance card and valid photo ID. Have your medication list and allergies ready, and tell the front desk about any change to your address, phone, insurance, or pharmacy.

Rescheduling

Let us know as early as possible if you need to move a visit. If you arrive more than 15 minutes late, you may be asked to wait for provider approval, see another available provider, accept a shorter visit, or reschedule.
